Hi!

I installed this yesterday. While testing I figured out, that only one radio button shows for the first option, there is none in the next line for the second option. Tried Edge and Chrome, same problem.
What could be the reason? I did everything as shown in your screenshots.

Edit: Same with checkboxes (only option in first line with checkobox) and dropdown (all 4 lines make one single option)
