If you followed the posts in this topic. I could not access Header Banner by clicking on its button, I got a General error message and so the Banner screen would not display. By chance, randomly I mistakenly clicked on Board Settings and behold the Header Banner configuration screen displayed. Something was not right.
Solution, in ACP deep in Mask, Administration Control Panel, there is Module Management listing the modules that control the actions of the buttons. Please use the link above to read about it.
The upgrade to 3.3.11 is complete. It turned out there was no corruption. phpBB3 provides a place to change the target assignment of the buttons in Module Management. And it was there that a prior administrator re-targeted the buttons and incorrectly programed the Header Banner resulting in a General Error message.
This journey would not have come to a successful conclusion without the forum member warmweer. It was he who led me to Module Management and I am gratefully thankful.

It turns out there was no corruption causing the button behavior nor other behaviors in the ACP. The database had no issues either. Although, since no one could explain why the issue was happening it was assumed there was some type of corruption in phpBB3 and/or the database. It turns out phpBB created Module Management to effect behaviors and allows the Administrator to change every button's target* on the General tab in the category Board Configuration. This is where the button Board Settings was set to access Header Banner. And, the button Header Banner was improperly executed causing a General Error message. Entering the appropriate information the buttons returned to the expected behavior and work well.
One can find the screen Module Management at: ACP; System; Administration Control Panel; General; Board Configuration; Module Management.
