This is what I've done in the meanwhile.
-Disabled all extensions one by one, it was pain. for each I got at least 15 minutes of high CPU usage. Then i got same for deleting data of each extension. It took hours to complete that part.
-Updated forum and database to the latest version
Next I installed fresh forum in parallel, emptied the db tables and copied data only into the new one then backed it up properly.
-I did complete wipe of the VPS, re-installed latest phpbb and restored the database
No high load, works perfectly
-Then I rebuilt search index (full text native) and start getting high CPU usage again but when I navigate trough the admin panel
Tried deleting cache - no effect
At this point i might just give up.
